About the Role
The purpose of the role is to ensure the best rehabilitation outcomes for our participants/workers by navigating and facilitating the access, management and cohesion of services and supports once their needs have stabilised post rehabilitation and settling into community life following serious injury. It will involve establishing, coordinating and monitoring community-based supports with a focus on maximizing the participants/workers’ independence and community engagement.

It’s an exciting time at icare where no two days are the same; as a Community Living Facilitator you will:
• Use a person-centred approach through supporting the participant/worker to develop a My Plan to maximise their involvement in planning through self-assessment, self-reflection and self-identification of strengths, priorities and goals in alignment with KPI’s of the role
• Assess and actively monitor the effectiveness of selected strategies and supports in meeting goals and steps throughout the My Plan period.
• Ensure an appropriate selection of supports in subsequent plans and to ensure the participant/worker is making future decisions based on comprehensive information about effectiveness of past choices as supported by internal specialist services.
• Apply critical reasoning and thinking within a decision-making framework within the team and in collaboration with internal specialist services and use of resources when establishing and coordinating requests for services and supports to ensure they appropriately meet the needs of people with severe injuries in terms of their goal for independence and community engagement.
• Approve recommendations and funding regarding reasonable and necessary treatment, care and support within agreed functional delegations
• Engage immediately in the event of a serious incident or risk to the participant/worker’s safety with the necessary support to ensure the safety and wellbeing of the participant/worker’s wellbeing.
• Adhere to the KPI’s that monitor and measure key areas of the role
• Keep abreast of and understand the legislative framework and Guidelines and how these contribute and impact the decision-making framework.
• Contribute and support the knowledge management of research and advice on injury management and service delivery to people with severe injuries to identify gaps and barriers in service delivery and share this broadly across the business.
• Work within the team to ensure the appropriate provision of services for people with severe injuries in a cost-effective manner.
• Work collaboratively with all teams across Care, in the design and implementation of internal quality assurance management of disputes and complaints and continuous improvement systems to drive quality and consistency in decision making.
• Maintain relevant records and manage service approvals and payments in accordance with agreed needs, assessments and plans.
• Liaise and collaborate with service providers to ensure the optimal delivery of programs to people with severe injuries.
• Identify and participate in own professional development opportunities to ensure currency of skills and knowledge with regard to current evidence and best practice.
